FLU Shots

Scheduling: We will begin giving flu vaccine on 9/4/12, on Tuesdays and Thursdays.

Appointments are required for flu vaccine.

Influenza season in the Bay Area is typically December and January.

This year’s vaccines combine H1N1 vaccine with 2 strains of seasonal flu vaccine.

Who needs it?

Adults age 50 or over

Children and teens ages 6 months to 18 years (until 19th birthday)

Adults for whom it is medically necessary (see below)

Pregnant women (at any stage of pregnancy)

Residents of nursing homes and other chronic care facilities

Household members and caregivers of infants under the age of 6 months

Healthcare workers providing direct, hands-on care to patients (should try to get from employers first)

What is medical necessity?

patient has one of the following problems

or is primary caregiver for someone with one of the following problems

or lives with someone with one of the following problems:

anemia, congenital disease, heart, diseaseasthma, COPD, immunodeficiency, cancer, diabetes, renal disease.

Coverage:

The following health plans pay for flu shots: Medicare, Alameda Alliance plans, Medi-Cal, HMOs administered by Hill Physicians

For patients with all other insurance plans, we will bill your insurance. If the flu shot is not covered, we will bill you for both the vaccine and the nurse visit ($72.) But, if you pay for it at the time of service, and waives billing insurance the charge will be just $30. Some other plans cover flu shots only for patients over 65 or with medical necessity (see above), and many insurance plans do not pay for flu shots even with medical necessity.

Our team will continue to care for you at our existing office with the same providers and staff. Availability will remain the same. Access to your community specialists and local hospitals will not change. We will continue to preserve the same personalized private practice environment while providing access to one of the world’s leading medical institutions.

We believe this affiliation will ultimately enhance the services and outcomes we provide our patients. We will gain greater technological abilities and will implement an electronic health record which will enable us to offer patients more tools to communicate with their physicians and our team online. Travel Services at EBFP

EBFP does not provide expert travel advice and recommendations. We can, however, take care of most of your travel needs.

We recommend that you begin by going to the Center for Disease Control Website

(http://wwwnc.cdc.gov/travel/destinations/list.htm) and click on your country of destination and review the very specific list of recommended vaccines and malaria prevention.

You can then schedule an appointment at EBFP (please bring the CDC list with you). We can provide all immunizations with the exceptions of rabies, Japanese encephalitis and yellow fever. We can also write you a prescription for the appropriate anti-malaria drug. We can not write travel prescriptions without an appointment.

Not all medical insurance companies cover an office visit or immunizations for travel related services. The cost of the an office visit fee for travel related services is $75.00 plus the cost of immunizations. We ask that you pay us for the visit and immunizations at the time of service. We will, as a courtesy, bill your insurance company for you, sometimes they will cover this visit and/or the immunizations. If they do we will promptly send you a refund.

Notice to Medicare Patients

In 2011, Medicare is providing a new benefit called the “Wellness Visit.” It is not a routine, annual complete physical exam. Medicare still does not cover a complete physical.

The Wellness Visit only includes:

Personal and family medical history

Height, weight and blood pressure

Detection of any cognitive impairment

Review of potential risk factors for depression

Review of functional ability and level of safety

A written screening schedule for preventive services

Personalized health advice and a referral to services or programs to reduce health risks and promote wellness

(Voluntary) advance care planning

We encourage you to take advantage of this extra time with your practitioner. There is no cost to you for this appointment.

The Wellness Visit does not replace a complete annual physical exam. If you would like a Wellness Visit, please tell the person who is scheduling your appointment.

Parking Procedure

As of August 2010, there has been a new parking procedure for our office building. All cars will be valet parked from the front entrance on Telegraph Avenue. There will be a $2 fee for parking.
